Michigan Correction of Errors

Should your commission certificate contains an error, you must immediately notify the Office of the Great Seal at 517-373-2531 or by completing the "Notice of Change Form." There is no need to reapply, and there is no charge to correct our records.

Michigan Change of Address (within same county)

To change your address within the same county, you must complete and submit online a Notaries Change of Address form listing your name (as commissioned), old address, new address, county (where commissioned), and dates of birth and expiration for identity purposes. There is no charge to change an address in the same county.

Michigan Change of Name or Address (to a new county)

If you legally change your name or your address to a new county after you have been commissioned as a notary public, or if your commission certificate contains an error, you must immediately notify the Office of the Great Seal at 517-373-2531 or by completing the "Request for Duplicate/Notice of Change Form." There is no need to reapply, and there is no charge to correct our records. If your certificate has been lost or destroyed and you would like to replace it, you may request a duplicate by submitting a written request to The Office of the Great Seal with a $10 processing fee.

Michigan-Notary Express Change of Name Service

Return the name change request to our office and we will provide you the following:

Process change request with State of Michigan
Provide you with a New Self-inking Notary Seal with new name
Obtain certificate with new name.
