Submit Application, Oath, Exam


After purchasing your notary package, please submit the following by either Email or Mail to the Wisconsin Secretary of State:

 

  • Your signed notary application.
  • Your signed Oath of Office form
  • Your certificate from passing the Notary Exam with 90% or better (taken within 1 year of the date of submission of the application)
  • Your notary bond information
  • Email all forms to: DFINotary@dfi.wisconsin.gov

Wisconsin Notary Qualifications


To become or renew as an WISCONSIN Notary Public, you must:

  • Be at least 18 years of age

  • Be a resident of the State of Wisconsin

  • Be a citizen of the United States

  • Have at least the equivalent of an eighth grade education

  • Not have been convicted of a felony or a misdemeanor violating the public trust, unless the conviction has been pardoned

  • Be familiar with the duties and responsibilities of being a Notary Public
